    Northwest Permanente, P.C. is currently seeking Board Certified or Eligible Internists, Family Practitioners or Geriatricians to provide comprehensive Primary Medical care to home-bound Kaiser Permanente members. Our innovative practice model focuses strongly on coordinated care, emphasizing transitions, end-of-life care and an individual's values. The successful applicant will be a dynamic and committed team player who is skilled at the management of complex and frail patients.


    • Manage and deliver home-based primary care to a small panel (100-120) of complex, homebound patients in a supportive, team-based care model that includes that includes a social worker, RN, Medical Assistant or LPN, geriatric psychiatrist and pharmacist.
    • Enjoy some flexibility in planning your schedule to provide exceptional care to your paneled patients, while striving for work-life balance
    • Care for 4 to 5 patients per day, with the time to practice high quality medicine, manage your Inbasket and review new patients or difficult cases.
