  • Overview

    Northwest Permanente, P.C. is currently seeking Board Certified or Eligible Internists, Family Practitioners and Geriatricians to work as a locums tenens provider with a group of experienced and passionate clinicians who provide comprehensive primary medical care to home-bound Kaiser Permanente members. Physicians will manage a panel of approximately 100 patients who reside in a variety of community-based settings (e.g. assisted living, memory care, adult foster homes and private residences). Physicians serve an integral role on an interdisciplinary team which includes social workers; nurse case managers and pharmacists. Our innovative practice model focuses strongly on coordinated care, emphasizing transitions, end-of-life care and an individual's values.

     

    Position responsibilities also include participation in after-hours phone call and weekend nursing home rounding on a rotating basis which is shared across a large clinician group. The successful applicant will be a dynamic and committed team player who is skilled at the management of complex and frail patients.
