Northwest Permanente, P.C. is currently seeking Board Certified or Eligible Palliative Medicine physicians to join our growing medical group. This physician will be part of a 13-physician/provider team in Portland, Oregon. Board certification or eligibility in Hospice and Palliative Medicine is required. Fellowship training is desired. Candidates should have excellent communication, collaboration, and primary care medical skills. This opportunity will specifically focus on our Inpatient consultation service. Our Inpatient Palliative Care programs have become integral to the care of patients with complex illnesses and are very well received by referring providers, patients and families. In addition, for those interested, there is ample opportunity to become involved in the continuing education of nurses and hospitalists.
This is an exciting time to work in the department of hospice and palliative medicine at Northwest Permanente. The opportunities for palliative care clinicians are challenging and varied, and there is broad institutional support for our mission. As a member of our group, you will practice in a supportive and collegial environment, as an essential member of the care team. We have our own hospice program, integrated clinic-based palliative care and home-based palliative care, as well as an inpatient consultation service. Our programs are comprehensive, well-rooted and under a unifying umbrella of Kaiser Permanente. A major advantage of our contained system is the ability to provide seamless transitions for our palliative care patients with tight interfaces between primary care, inpatient care, home health and geriatric services. The common electronic medical record and electronic prescribing are key to the quality of care we provide our patients.
The mission of the Hospice and Palliative Care team is to provide comprehensive care to patients with serious life limiting illness, focusing on the multiple aspects of suffering: physical, emotional, psychological and spiritual. The team at Sunnyside Medical Center is staffed by a physician, nurse practitioner, medical social worker and palliative care nurse. This is a high-volume consult service seeing patients on medical/surgical units, ICU/CCU and neurology units. The Westside Medical Center is staffed by a physician, medical social worker and nurse with time divided between morning clinic and an afternoon Inpatient consultation service. Both inpatient teams can provide ongoing support during the entire hospitalization with focus on symptom management and psychosocial issues. To best meet the spiritual needs of our patients, there is also close collaboration with hospital chaplaincy.
